Applying for the Canadian Dental Care Plan (CDCP) is straightforward, but you’ll need a few key documents to verify your eligibility. Having these ready will help speed up the application process through the CRA.
Here’s what you’ll typically need:
🧾 1. Government-Issued ID
A valid ID is required to verify your identity and residency status in Canada. This can include:
- Driver’s licence
- Health card
- Passport or permanent resident card
💼 2. Proof of Income
The CRA uses your most recent tax return to verify your family net income. Ensure you’ve filed your taxes for the most recent year.
You may also need your Notice of Assessment.
If applying for a child, you’ll need the parent/guardian’s income details.
🧑🦽 3. Disability Documents (If Applicable)
If applying as an adult with a disability, you may need to provide:
- A valid Disability Tax Credit (DTC) certificate
💻 4. CRA My Account Login
You’ll apply for CDCP through your CRA My Account. If you don’t have one yet, you’ll need to register and verify your identity.
