- Look for a clinic that advertises CDCP acceptance
- Call ahead to confirm they participate in the plan
- Book your appointment and bring your CDCP dental card
There’s no need to get a referral or notify your old clinic unless you’re transferring records.

To maintain continuity of care:
- Ask your previous clinic to forward your dental records (you may need to sign a release form)
